Skilled Nursing; LPN or RN Phoenixville

We are seeking licensed practical nurse or registered nurse to work one‑on‑one with our pediatric client in a home setting in the Phoenixville, PA area. Available shifts are Monday through Friday or Saturday and Sunday. Additional clients in the surrounding area and full‑time or part‑time positions are also available.

• Must be a caring and reliable individual who can work in a diverse atmosphere
• Assess patient’s need for safety measures and appropriate interventions
• Implement the nursing care plan as outlined by the primary nurse
• Implement medication changes according to verbal and/or written physician orders
• Administer prescribed medications and treatments
• Accurately observe and promptly report changes in the patient’s physical or mental status to the nursing supervisor
• Accurately record nursing observations, activities, and treatments on the appropriate forms
• Follow a physician’s plan of care under the supervision of our Director of Nursing
• Communicate with family members, office staff, and all supporting health professionals

• Current Pennsylvania skilled nursing license
• Current CPR certification
• Must be able to lift at least 50 pounds
• Must pass state and federal criminal background checks
• Must have up‑to‑date physical and two‑step PPD or chest X‑ray
• Must be able to pass skilled nursing pre‑employment test
• Form of transportation to client’s residence
