Monogram Health is seeking a Care Manager – Registered Nurse to lead in-home care management for patients with chronic kidney disease and ESRD. You will collaborate with physicians, advanced practice providers, and social workers to develop and adapt care plans, perform home visits, monitor health data, and educate patients and families.
Must have an RN license, 2+ years in care management or CKD/ESRD, and willingness to travel locally; BLS certification is required with onboarding support.
