Job Description Summary

Ohio Health Palliative Care Clinic is seeking an experienced Registered Nurse to join our innovative, team‑based care model. The role supports patients with advanced cancer by providing compassionate, longitudinal care focused on symptom management, education, and quality of life.

Schedule:

Monday – Friday, no weekends or holidays.

Hours:

8:00 a.m. – 4:30 p.m.

Responsibilities and Duties
  • Provide clinical care within RN scope of practice and document assessments.
  • Document moderate to distressing symptoms to Palliative Physicians, APPs, and floor nurses.
  • Follow up on hospice GIP patients as needed.
  • Participate in advance care planning discussions and complete documentation of advance directives.
  • Communicate care plans to referring providers, unit clinical teams, and ensure recommendations are implemented.
  • Educate patients, families, and hospital staff about palliative care.
  • Serve as a resource and support to patient care units, including medication delivery and end‑of‑life care.
  • Identify learning needs of patient care staff and communicate these to Palliative leadership.
  • Maintain professional communication with providers, hospital staff, patients, families, internal and community resources.
Minimum Qualifications
  • Bachelor’s Degree (required)
  • Experience in a related clinical area such as oncology, geriatrics, ICU, or neuro in an academic or large community hospital (3‑4years).
  • 3‑4years of palliative care or hospice experience.
  • CHPN specialty certification (preferred).
Specialized Knowledge
  • Knowledge of palliative care practices and interdisciplinary teamwork.
Work Shift

Day shift, 40 scheduled weekly hours, Department:
Palliative Care.
