Summary

Job Title: Nurse Navigator - Breast Health
Location: 1700 Reisterstown Road, Suite 102, Pikesville, MD 21208

Job Summary:

We are seeking an empathetic and experienced Nurse Navigator to join our Breast Health team. This role provides community-based care navigation, offering essential education and guidance to high-utilizer patients. The Nurse Navigator will collaborate closely with the healthcare team—including nurses, social workers, community health workers, and physicians—to develop and implement personalized care plans. You’ll play a critical role in ensuring patients receive the best possible care, both within the healthcare system and in their communities.

Key

Responsibilities:
  • Multidisciplinary Care Coordination:

    • Develop individualized, multidisciplinary care plans for patients in collaboration with physicians and other healthcare team members.

    • Provide navigation support, helping patients understand and access healthcare and community resources to ensure seamless care and follow-up.

  • Patient Advocacy & Support:

    • Advocate for patients, families, and caregivers, addressing their unique needs and ensuring they have the support required to navigate their healthcare journey.

    • Assist patients with access to services, including in-person visits to the hospital, Emergency Department, physician offices, patients' homes, and over the phone.

  • Nursing Assessment & Disease Management:

    • Conduct comprehensive nursing assessments to determine the strengths and needs of patients and their families in managing disease processes.

    • Assist patients and caregivers in effectively managing their health and treatment regimens.

  • Quality Improvement & Data Reporting:

    • Gather and analyze data related to patient care, providing feedback and reporting to ensure continuous quality improvement and growth of the program.

    • Participate in program development and implementation, enhancing resources to support high-risk patient populations.

  • Community Program Development:

    • Contribute to the development and expansion of the community patient navigation program, ensuring it meets the needs of diverse patient populations.

    • Collaborate with the multidisciplinary team to design new initiatives that address the evolving needs of high-risk patients.

Qualifications:
  • Required:

    • Current Registered Nurse (RN) License in Maryland or eligibility to obtain Maryland license.

    • Basic Life Support (BLS) certification required.

    • Oncology experience is required.

  • Preferred Skills:

    • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and a multidisciplinary team.

    • Ability to navigate complex healthcare and community support systems to improve patient outcomes.

    • Experience with care coordination, patient advocacy, and program development is a plus.
