As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home.
While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Position SummaryServes as a liaison between the organization and hospital staff, educating professional and non-professional team members on the organization’s services and programs. Facilitates effective communication and collaboration with physicians and interdisciplinary teams within the inpatient setting. Leads the development, implementation, monitoring, and supervision of strategic initiatives aimed at increasing referrals. Assists with referral coordination and ensures a seamless transition of care.
Provides oversight and case management for hospitalized hospice patients. Reports directly to the Executive Director.
- Graduate of an accredited school of professional nursing.
- Current Registered Nurse (RN) license in good standing with the State Board of Nursing.
- Minimum of two (2) years of general nursing experience required; preference for experience in hospice, home health, medical-surgical, or critical care settings.
- Demonstrated competency in pain and symptom management.
- Skilled in professional nursing practice with the ability to support patients and families experiencing emotional stress; respectful and tolerant of diverse lifestyles and beliefs.
- Practices in accordance with accepted professional nursing standards and ethical principles.
- Demonstrates understanding of the organization’s philosophy and the principles of death and dying.
- Knowledgeable of Medicare Conditions of Participation for hospice programs and all applicable federal, state, and regulatory requirements.
- Maintains current CPR certification.
- Demonstrates strong administrative and organizational skills, including effective written and verbal communication and public speaking abilities.li>
- Ability to work independently with minimal direct on-site supervision.
- Proven ability to collaborate effectively with physicians, healthcare professionals, and interdisciplinary team members.
- Ability to travel locally and access hospitals, nursing facilities, and other healthcare settings as required.
- Valid driver’s license, reliable transportation, and automobile insurance in accordance with state and organizational requirements.
- Compliance with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
- Demonstrates strong oral and written communication skills, with the ability to convey information clearly and professionally.
- Possesses thorough knowledge of regulatory and clinical requirements related to the provision of hospice care.
