Summary:
The Social Service Director leads the Social Services Department and ensures our residents’ medical, emotional, and social needs are supported in accordance with state and federal guidelines.
Key Responsibilities:- Develop, implement, and monitor social service programs that meet all regulatory requirements
- Support discharge planning, resident assessments, and individualized social care plans
- Collaborate with residents, families, healthcare providers, and community organizations
- Coordinate department functions and manage social services staff
- Ensure compliance, resident rights, confidentiality, and quality standards
- Participate in quality improvement efforts and handle social services emergencies
- Bachelor’s degree in Social Work or a related field (required)
- Experience in long-term care or healthcare setting preferred
- Strong communication, problem-solving, and leadership skills
- Compassionate approach and ability to advocate for residents
