In-Home Health - Nurse Practitioner or Physician Assistant Colorado; Traveler

Position

Full‑time Clinician (Nurse Practitioner or Physician Assistant) – Signify Health, part of CVS Health®.

Provide In‑Home Health Evaluations to health plan members in the comfort of their homes, traveling up to 100% of the time across a defined coverage area. Bring a range of diagnostic and preventive services and use an iPad‑based clinical workflow.

• Conduct 45‑minute in‑home visits with patients, reviewing medication and medical history, performing physical evaluation, and ordering additional diagnostic tests as directed by the health plan.
• Use an iPad and the intuitive clinical workflow; not required to prescribe medication, order lab tests, or alter treatment regimens.
• Travel locally and within licensed states, up to 100% of the time; may include overnight hotel stays, car rentals, and flights.
• Maintain active, unrestricted licenses in all required coverage states and obtain multi‑state licenses as needed.
• Provide additional per‑completion incentive pay for all in‑home visits and conduct virtual visits when required.
• Collaborate with Signify Health clinicians and the Network Activation team to support licensure, renewals, and enrollment.

• Active, unrestricted nurse practitioner or physician assistant license(s) in the coverage area(s).
• Board certification required.
• Ability to obtain and maintain multi‑state licenses as required.
• Experience with in‑home or telehealth evaluations preferred.

• Experience in primary or preventive care.

Typical pay range: $87,035.00 – $, based on experience, education, geography, and other factors.
