The registered nurse/case manager plans, organizes, directs, and provides hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Hours / ScheduleFull Time; (40) hours / week;
Monday - Friday, 8:00am - 5:00pm
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State license as a Registered Nurse
- Current CPR certification
- Minimum of two years of experience as a Registered Nurse in a clinical care, home health, or hospice setting (preferred)
- Licensed driver with an insured automobile in good working order
- Excellent observation, verbal and written communication, problem‑solving, and basic math skills
- Proficient in nursing skills per the competency checklist
- Ability to lift, position, and/or transfer patients; lift supplies and equipment
- Case‑manage and provide clinical direction to the interdisciplinary team, physician, and family
- Implement, develop, and document the patient plan of care
- Provide care utilizing infection control measures that protect both staff and patients (OSHA)
- Assure continuity of quality patient care with appropriate documentation
- Monitor assigned cases to ensure compliance with third‑party payer requirements
- Demonstrate commitment, professional growth, and competency
- Promote agency philosophy and administrative policies
- Perform on‑call responsibilities and provide on‑call service to patients and families as assigned
- Supervise hospice aides and licensed practical nurses
- Perform other nursing duties as assigned by management to support department goals and initiatives
