Responsibilities- Assess, monitor, and document residents' wounds and skin conditions
- Provide wound care and treatments according to physician orders and established care plans
- Monitor residents for changes in condition and notify the appropriate members of the care team
- Maintain accurate and timely clinical documentation
- Collaborate with physicians, nursing leadership, and the interdisciplinary care team
- Educate residents, families, and staff on wound prevention and treatment
- Assist with developing and updating individualized wound care plans
- Promote infection prevention and ensure compliance with facility policies and applicable regulations
- Provide compassionate, resident-centered care
- Active North Carolina LPN or RN license
- Wound care experience required
- Long-term care experience preferred
- Strong assessment, communication, and documentation skills
- Basic leadership and supervisory skills
- Dependable, compassionate, and team-oriented
- Scheduling flexibility as needed
