Position Summary

The Social Worker / Discharge Planner is responsible for supporting residents’ psychosocial needs while coordinating safe and efficient discharge planning. This role works closely with the interdisciplinary team to promote positive outcomes for residents and their families.

Key Responsibilities
  • Conduct psychosocial assessments and develop individualized care plans
  • Coordinate discharge planning from admission through transition
  • Collaborate with nursing, therapy, physicians, and families to ensure smooth care transitions
  • Assist residents and families with community resources and post-discharge services
  • Advocate for resident rights, dignity, and independence
  • Participate in care plan meetings and interdisciplinary team discussions
  • Maintain accurate documentation in compliance with state and federal regulations
  • Provide emotional support and counseling as needed
Qualifications
  • Previous experience in skilled nursing or healthcare setting preferred
  • Knowledge of long-term care regulations and discharge processes
  • Strong organizational and communication skills
  • Compassionate, resident-centered approach
