Key Responsibilities
  • Patient Care: Provide direct nursing care, administer medications, treatments, and monitor vital signs during weekend shifts; assist residents with activities of daily living (bathing, dressing, feeding, mobility); respond promptly to resident needs and emergencies; monitor conditions and report changes to supervising RN or physician.
  • Medication Administration: Administer medications as prescribed, ensuring accurate documentation, monitor for adverse reactions, report concerns to medical team, maintain accurate medication records in compliance with policies.
  • Documentation and Compliance: Accurately document all patient care activities in the EHR system; ensure compliance with federal, state, and facility regulations; participate in shift handovers, updating incoming staff on resident conditions.
  • Resident Advocacy and Support: Provide emotional support, companionship, ensure comfort and dignity; advocate for residents' needs with care team; communicate effectively with families.

    Collaboration:

    Collaborate with nursing team, CNAs, and other healthcare professionals; participate in care planning meetings; assist with weekend admissions, discharges, and transfers.
Qualifications
  • Education: Graduate of an accredited LPN program.
  • Licensure: Current and active Licensed Practical Nurse (LPN) license.
  • Experience: Previous experience in a healthcare, long‑term care, or assisted living setting is preferred but not required.
  • Skills: Strong clinical and assessment skills; excellent communication and interpersonal skills with a compassionate approach; ability to work independently and manage multiple priorities in a fast‑paced environment; proficiency in using EHR systems and maintaining accurate documentation.
