Job Description

Duties include but are not limited to:

• Determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.
• Provides gender-specific direct veteran care and care coordination, as it pertains, to veterans and their care within the primary care department.
• Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
• Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
• Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
• Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the veteran's goals of care.
• Evaluates practice in an ongoing process, based on best evidence.
• Provides peers with formal or informal constructive feedback for improvement.
• Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care.
• Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
• Responsible for the documented PACT outcomes at the program or service level.
• Performs other duties as assigned.
