Job Summary
Trinity Home Health is a boutique home health agency dedicated to providing high-quality, patient-centered care in the comfort of our patients’ homes. We are currently seeking an experienced Registered Nurse (RN) to join our team on a full-time or part time basis in the San Luis Obispo North County region.
The Home Health RN will be responsible for delivering skilled nursing care, coordinating patient care plans, and collaborating with an interdisciplinary team to ensure excellent outcomes. This role is ideal for a self-motivated nurse who values autonomy, flexibility, and meaningful patient relationships.
Key Responsibilities
- Perform skilled nursing visits in patients’ homes in accordance with physician orders and plan of care
- Complete comprehensive assessments, ongoing evaluations, and accurate clinical documentation
- Administer medications, wound care, injections, and other skilled nursing treatments
- Educate patients and caregivers on disease processes, medications, and home care techniques
- Coordinate care with physicians, therapists, and other members of the care team
- Monitor patient progress and promptly report changes in condition
- Ensure compliance with home health regulations, agency policies, and standards of care
Qualifications
- Active California Registered Nurse (RN) license
- Minimum of 1 year of home health nursing experience required
- Current CPR certification
- Strong clinical assessment and critical-thinking skills
- Excellent communication and documentation abilities
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver’s license
- Familiarity with OASIS and EMR systems Required
