Case Manager II

About the roleWe are searching for a Case Manager II who will support programs for survivors of domestic violence and Permanent Supportive Housing participants. The role includes frequent travel for home visits and requires compassion to make a lasting difference.

Employment DetailsFull Time, Non‑Exempt, Monday – Friday during business hours
Compensation$30–32 per hour (grant funded, may expire December 2026)
Intake and AssessmentScreen prospective participants for eligibility, provide program information, obtain required consent documents, and complete assessments (HMIS Intake, VI‑SPDAT, Housing Retention Assessment) to evaluate strengths, needs, and risk indicators.
Case Planning and MonitoringConduct interviews, gather resources, formulate individualized plans, establish goals, document in the record system, communicate plans to support staff, implement follow‑ups, reassess participant progress, adjust plans, promote appropriate referrals, maintain accurate documentation, liaise with internal and external service providers, manage a caseload of up to 25 households, assist with budgeting and financial resources, ensure children enroll in school, report challenges to supervisors, prepare internal and external progress reports, and attend required trainings.

Open service files, keep HMIS data up‑to‑date, maintain case files per guidelines, ensure data integrity and security, and meet data quality standards.
What you’ll bring- College degree in social services or related field or equivalent education/experience;
Associate's with relevant experience considered. - Experience with vulnerable populations, including domestic violence survivors.
- Proficient computer skills;
Microsoft Office Suite. - Flexible schedule for evening/weekend visits.
- Bilingual language skills are a bonus.
- Knowledge of social service resource systems and self‑help interventions.
- Counseling experience with mental health, substance abuse, and homeless populations.
- Public benefit and financial resource knowledge.
- Motivational interviewing and trauma‑informed care skills.
- Family budgeting and money‑management skills.
- Data collection, entry, and quality control in web‑based databases.
- Professional ethics, confidentiality, integrity.
- Strong oral and written communication and listening skills.
- Self‑motivated, accountable, and organized.
- Frequent driving; valid California driver license, reliable vehicle, and auto insurance.
- Background check and TB clearance required.
