Family Preservation Services Case Manager

Starting Salary: $45,000 Annually

The Family Preservation Services (FPS) Case Manager works in the home of families to teach problem-solving, crisis management, parenting, communication, budgeting, home maintenance, life skills, and more. The role connects families to community resources to stabilize them. A member of the Family Preservation team, reporting to the Manager of Family Preservation.

Responsibilities
  • With assistance from a Family Support Worker, carry a caseload of 12-15 families.
  • Meet at least weekly with families in the home and with each child alone at least monthly to address safety, well-being, and progress on case plan activities.
  • Be available to help families with any crisis or conflict that might arise.
  • Develop Family Case Plans (and Child Case Plans when necessary) and coach families toward completing case-plan tasks.
Qualifications
  • Bachelor's degree in social work or a related human services field is required.
  • Master's degree is preferred.
  • At least 21 years of age and must pass background check, physical, and drug screening.
  • Valid driver's license in the state of residence, proof of current vehicle insurance, and reliable transportation.
  • Minimum two years of relevant experience working with children.
