ISO Support Service Worker/Caregiver – In Home and Community

We are currently seeking responsible, caring, and dedicated people to fill ISO Support Service Worker positions in our Home and Community Services (HCS) program across Lawrence County. ISO Support Service Workers play a crucial role by providing care and supervision for daily living, social, and recreational activities. They ensure the safety and well-being of individuals and serve as positive role models.

Services are provided in the individual’s home and/or community, and the ISO may use their personal vehicle for transportation when appropriate.

DUTIES AND RESPONSIBILITIES:

• Perform the tasks outlined in the individual's Individual Plan (IP).
• Complete assigned tasks as requested.
• Provide requested services within units authorized in the IP.
• Deliver services consistently with Passavant Memorial Homes policies and procedures.
• Provide supervision and positive role modeling at home and in the community.
• Complete required Pre‑Service and Annual In‑Service training.
• Observe behaviors and physical condition changes, reporting accurately.

REQUIRED SKILLS AND KNOWLEDGE:

• Minimum age 18.
• Valid driver’s license.
• Current car insurance and registration (position requires use of personal vehicle).
• Successful background check.
• Experience working with individuals with developmental disabilities (preferred).
