UPMC Home Health is hiring a full-time home care Physical Therapist to join our team! The person in this role would support New Castle and the surrounding areas.

Responsibilities:

• Perform duties in alignment with UPMC Home Healthcare’s service philosophy, focusing on patient-centered care. Continually develop professional skills through ongoing education.
• Participate in on-call coverage rotation for the agency. Demonstrate system-wide competencies and behaviors.
• Assign and supervise the Physical Therapy Assistant according to State Board guidelines.
• Consult with families, interdisciplinary team members, physicians, and other professionals to ensure optimal patient care.
• Transmit data (e.g., visit assessments, OASIS, 485, verbal orders) promptly, either the same day or as required.
• Complete all documentation accurately and promptly, including visit assessments, OASIS, 485, and verbal orders, in accordance with regulatory standards.
• Develop the plan of care collaboratively with the physician, patient, and UM professional, based on physician’s orders and patient needs, following the admission process.
• Evaluate, plan, and treat patients using diagnostic, muscle, nerve, joint, and functional ability testing within Physical Therapy Practice standards. Make recommendations and order necessary DME when indicated.
