In-Home NP​/PA Clinician — Part-Time Health Visits

Signify Health is seeking a Part Time Clinician (NP/PA) to provide in-home health evaluations. You’ll visit patients in their homes, review medical history, perform physical evaluations, and coordinate with health plans for additional tests as needed.

The role supports flexible travel within licensed states, virtual visits when required, and delivery of preventive health services. Board-certified NP/PA with active licenses is required.
