This is an in‑person RN position at both Geisinger Wyoming Valley and Geisinger South Wilkes Barre, providing on‑site support to inpatient care teams.

Job Duties & Responsibilities
  • At the heart of inpatient care coordination, ensuring patients receive the right care, at the right time, in the right place.
  • Collaborate closely with interdisciplinary teams while making a meaningful difference in outcomes that matter to patients and families.
  • Provide on‑site support to inpatient care teams in a fast‑paced, mission‑driven environment.
  • Typical work week Monday through Friday, 8:00AM‑4:30PM; occasional coverage of approximately every third weekend.
  • Assess, plan, implement, coordinate, monitor, and evaluate all options and services to optimize patient or member health status.
  • Manage utilization and practice metrics to refine care delivery models for maximum clinical, quality, and fiscal outcomes.
  • Integrate evidence‑based clinical guidelines, preventive protocols, and other metrics into patient‑centric treatment plans.
  • Develop systems of care that monitor progress and promote early intervention in acute care situations.
  • Assist with design, implementation, and evaluation of the advanced patient‑centered care model.
Qualifications
  • At least 2 years of prior RN experience in a hospital setting.
  • Prior case‑management experience preferred (not required).
  • Must be available to work on‑site at Geisinger Wyoming Valley (GWV) or Geisinger South Wilkes Barre (GSWB).
  • This role is exempt and salaried.
Education

Graduate from a Specialty Training Program in Nursing (Required).

Experience

Minimum of 2 years in nursing (Required).

Certification & License

Licensed Registered Nurse in the State of Pennsylvania –  of Pennsylvania.
