Job DescriptionCase Manager RNs are responsible for the following:
- Advocates for the patient and family throughout the entire episode of care.
- Monitors patient’s progression intervening as needed to ensure efficient patient progress.
- Coordinates all aspects of discharge planning and establishes a safe discharge plan.
The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Qualifications- Bachelors of Nursing required
- Three (3) + years case management/discharge planning experience
- CCM certification preferred
- Active RN license in NJ/PA
- Comprehensive Medical, Dental, & Vision Plans
