Overview4 10‑hour shifts (8a.m.–6p.m.) Friday–Monday. Preferred candidates will live within 50miles of the Pittsburgh or Erie, PA region.
Responsibilities- Serve as the consistent point of contact for referring physicians, patients, caregivers, families, and the community.
- Facilitate patient flow, including coordination of appointments and assistance with accessing clinical and supportive care services within the Network.
- Coordinate all aspects of care in collaboration with the multidisciplinary team for all adult patients within specialized disease states to ensure quality and comprehensive services.
- Identify community needs and provide education, screening, support, referrals, and coordination of care as necessary.
- Oversee community outreach, high‑risk populations and disease‑specific sites to establish and sustain working relationships within the network.
- Educate and coordinate care regarding patient's diagnosis, treatment options, clinical trial information, and available resources.
- Work with the health‑care team to ensure safe handoff and coordination of care between facilities and between inpatient and outpatient settings.
- Collaborate with High‑Risk Care Teams to reduce re‑admissions.
- Assist Registry staff with data collection, patient outcomes, and updates to care delivery models.
- Coordinate appointments including all aspects of the multidisciplinary team and accompany patients to appointments as needed.
- Ensure medication adherence issues are addressed.
- Identify at‑risk patients and facilitate appropriate screening processes in partnership with AHN physician offices/facilities.
- Trend data and outcomes as established for the navigation program and identify gaps to improve patient care across the continuum.
- Develop or attend existing clinical care conferences to report on active patients to the multidisciplinary team.
- May identify bereavement needs of families and develop a plan of care.
- Perform other duties as assigned or required.
- Current State of PA RN licensure or current multi‑state licensure through the enhanced Nurse Licensure Compact (eNLC).
- 1–3years nursing experience in the required specialty.
- CPR certification—American Heart Association.

33 Child Abuse Clearance Certificate, Act
73 FBI Fingerprinting Criminal Background Clearance Certificate. - Bachelor’s Degree in Nursing (preferred).
- Certification in area of expertise (preferred).
- Valid PA driver’s license (may be required).
- Older Adults Protective Services Act (OAPSA) clearance may be required depending on location.
Pay Range Minimum: $34.29 – Pay Range Maximum: $54.25
