Home Health RN Case Manager

Paramount Home Health is seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join our growing team. This facility-based home health role provides care to patients residing within three nearby Paramount assisted living communities in the South Hills of Pittsburgh, minimizing travel while allowing you to make a meaningful impact on patient outcomes. If you are passionate about delivering high-quality patient care and enjoy working in a supportive, team-oriented environment, we'd love to hear from you.

What You Will Do:

• Visit patients for evaluations and skilled nursing care
• Write and update visit notes in Brightree within 24 hours
• Take part in team meetings and training sessions

What You Need:

• Active Pennsylvania RN license
• Experience with OASIS (helpful, but not required)
• At least 1 year of home care experience (preferred)
• Reliable transportation and proof of insurance
• Ability to travel to Paramount's three assisted living facilities located in the South Hills of Pittsburgh.
