Blended Case Manager
The Blended Case Manager with Crawford County Human Services provides advanced social services work to children and adults with serious and persistent mental illness. You will help individuals work through challenges, build connections, and guide them toward stability.
Responsibilities- Serve as a mandated reporter at all times.
- Provide intensive case management services to individuals with serious and persistent mental illness.
- Maintain required billable service units and enter monthly data by the established deadline.
- Complete daily documentation such as forms, case notes, reports, and correspondence.
- Obtain information from consumers, families, and others to complete mental health assessments.
- Develop and implement individual service plans based on assessments and interviews.
- Full‑time employment: 35 hours per week.
- Work hours:
8:30 AM–4:30 PM, Monday–Friday, with a 60‑minute lunch. Occasional overtime and travel may be required. - Telework: not available.
- Salary: starting salary may be non‑negotiable depending on the case.
- Minimum two years of experience as a County Caseworker 2; or
- A bachelor’s degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and three years of professional social casework experience in a public or private social services agency; or
- Any equivalent combination of experience and training which includes 12 college credits in the aforementioned social sciences.
- PA residency requirement is currently waived for this title.
- You must be able to perform essential job functions.
- Must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law. A conditional offer of employment will require submission and approval of satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI fingerprint clearance.
