DESCRIPTION OF WORK

This position focuses on protecting older adults and supporting their ability to live safely and independently. It combines investigation, communication, documentation, and coordination with a wide range of partners. As an Aging Care Manager 3, you will perform the following duties:

• Case Investigation:
  Conduct investigations of suspected abuse, neglect, abandonment, or exploitation
• Information Gathering:
  Collect medical, psychological, and financial information to support service eligibility decisions
• Case Documentation:
  Prepare case histories, write reports, maintain complete records, and update information in the Well Sky system
• Collaborative Support:
  Work with families, social workers, physicians, courts, attorneys, and law enforcement to address consumer needs
• Service Coordination:
  Participate in joint visits, conferences, and meetings to plan and deliver appropriate services
• Court Preparation:
  Recommend and prepare cases that require legal intervention, including documentation, testimony, and communication with attorneys

Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
• Telework:
  You will not have the option to telework in this position.

QUALIFICATIONS

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

Minimum Experience and Training Requirements:

• One year as an Aging Care Manager 2 or Aging Program Assessor; or
• One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

• Hiring preference for this vacancy may be given to candidates who live within Lehigh County. If no eligible candidates who live within Lehigh County apply for this position, candidates who reside in other counties may be considered.
• You must be able to perform essential job functions.

• This position falls under the provisions of the Older Adult Protective Services Act.
• Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
