An exciting opportunity is available for a Full-Time Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Monticello, Minnesota. This role is ideal for both new graduates and experienced professionals looking to grow their careers in a supportive, flexible, and dynamic environment. New graduates are welcome to apply!
This position offers a variety of career growth opportunities, including professional mentorship, unlimited Continuing Education Units (CEUs), and the chance to work in a network with over 100 facilities across 11 states.
Key Responsibilities:
- Patient Assessments & Treatments:
Assess and treat patients based on individualized care plans to meet their unique needs. - Documentation:
Accurately document patient progress, treatments, and outcomes in a timely manner. - Program Development:
Plan, market, and develop new programs to enhance patient care and improve overall outcomes. - Effective Communication:
Collaborate with patients, families, physicians, and healthcare professionals to ensure coordinated care. - Staff Education:
Lead in-service training sessions for fellow therapists and staff. - Clinic Operations:
Assist with the general operations of the therapy department, maintaining high standards of care and efficiency.
Qualifications:
- Licensure:
Current Physical Therapist (PT) licensure in Minnesota (or eligibility to obtain licensure). - New Graduates Welcome:

Strong communication skills with a compassionate, patient-centered approach to care. - Teamwork:
Ability to work independently as well as part of a collaborative healthcare team. - Interest in SNF Setting: A genuine interest in working within a Skilled Nursing Facility (SNF) environment.
