The Patient Placement Assistant prioritizes assignments for inpatient, observation, and day-stay patients, ensuring placement based on established criteria. Accurate documentation is maintained following all protocols, and the assistant acts as a liaison between Berkshire Health and external customers, ensuring customer satisfaction.
JOB SUMMARY- Under the direction of the Care Coordination Supervisor, the Patient Placement Assistant receives and assesses requests from all major customer constituencies: patients, providers, and service areas, including the transfer center. The role involves analyzing and coordinating the exchange of accurate and thorough information to ensure proper patient placement.
- This position works closely with the Care Coordination Supervisor to coordinate patient placement functions and assists in collecting key data elements for departmental metrics and monitoring.
- Experience:
- Proven customer service experience required.
- Direct Patient care experience preferred.
- Education:
- High School or GED required.
- Associate’s degree preferred.
- Licensure &
Certifications:- None
- Strong verbal and written communication skills, enabling effective interaction with patients, healthcare providers, and external service areas.
- Ability to prioritize and manage multiple tasks, ensuring timely patient placement and coordination while adhering to quality standards.
- Ability to assess and analyze patient placement requests to ensure accurate decision-making based on established clinical criteria.
- Ensure accurate documentation and follow established protocols.
- Strong teamwork skills, working effectively with the Care Coordination Supervisor and other healthcare staff to ensure smooth patient flow.
- Demonstrate a service excellence attitude, ensuring that all calls and interactions are professional, respectful, and meet the needs of patients, families, and staff.
- Ability to adapt to changing priorities and work effectively in a fast-paced environment.
- Proficient in using computer systems and patient placement software to manage patient placement tasks efficiently.
- Ability to assist in the collection, management, and reporting of key data elements for departmental performance monitoring.
- Ability to synthesize clinical information to properly place patients in the correct location.
- Understanding of patient confidentiality standards and adherence to hospital policies to maintain privacy and compliance.
- Proficient in computer applications.
