Community Health Programs is a network of health centers that provide primary and preventive care. Being in a federally designated rural community and Medically Underserved Population Area, CHP offers comprehensive services including health screenings, insurance enrollment assistance, and wellness education.

Description

The Behavioral Health Clinician advocates for clients, helping them access resources and address mental illness, addiction, and abuse. They support CHP’s mission and comply with policies and procedures.

Essential Duties & Responsibilities
  • Advocate and help clients gain access to resources while addressing personal issues such as mental illness, addiction, and abuse.
  • Work closely with all members of the healthcare team in developing the Plan of Treatment, including discharge plans, based on psychosocial assessment and accepted social work interventions.
  • Develop and maintain thorough knowledge of referral resources throughout the service area.
  • Adhere to all third‑party payer requirements.
  • Maintain accurate and timely records, including clinical progress notes, in accordance with facility policy.
  • Provide positive customer relations with patients, families, primary care providers, and other members of the healthcare team, referral sources, and service agencies.
  • Adhere to all department and facility policies.

Essential

Skills and Qualifications
  • Education:

    Master’s degree in Social Work.
  • License: LICSW or LMHC professional license per MA state law.
  • Experience:

    One (1) year full‑time clinical experience.
