Overview

The Care Coordination Supervisor collaborates closely with the Patient Placement Assistant, and the Patient Placement Coordinator in admitting, to coordinate patient admission and transfer requests from the Emergency Department (ED) and Transfer Center across multiple facilities. This role also manages internal patient flow, facilitating movement between levels of care and bed assignments to ensure that patients receive care in the most appropriate setting.

Responsibilities
  • Assess patients' clinical needs and ensure accurate placement within Berkshire Health Systems.
  • Integrate care management skills with clinical expertise to support patient care and the organization’s strategic goals.
  • Serve as the clinical expert and decision-maker for daily patient throughput and placement decisions, guiding the placement of patients to appropriate care settings.
  • Coordinate patient admission and transfer requests from the Emergency Department (ED) and Transfer Center across multiple facilities.
  • Manage internal patient flow and bed assignments to ensure patients receive care in the most appropriate setting.
  • Integrate clinical and financial components in placement decisions to align with organizational objectives, ensuring high-quality patient care and operational efficiency.
Qualifications
  • Experience:

    Five years of clinical nursing experience.
  • Education:

    Bachelor s Degree BSN
  • Licensure &

    Certifications:

    RN - MA RN License
Skills & Abilities
  • Familiar with community resources and utilization/quality review standards.
  • Ability to prioritize tasks and meet deadlines effectively.
  • Understanding and appropriately utilizing the hospital patient placement guidelines.
  • Strong decision-making and problem-solving in fast-paced environments.
  • Excellent communication and collaboration with healthcare teams and support staff.
  • Skilled in computer systems, phone communication, and patient placement software.
  • Ability to build strong working relationships with staff and external stakeholders.
