Registered Nurse, Case Manager

Job Summary

RN Full Time, 37.5 hours per week. No evenings, weekends, holidays, or on‑call. • Determine clinical eligibility for Mass Health‑funded services for nursing facility stays.
• Complete in‑home assessments of service needs for individuals.

• Current registered nurse with a valid Massachusetts license.
• Associate of Science in Nursing and at least one year of clinical experience in a community or long‑term care setting, or Bachelor of Science in Nursing.
• Geriatric/Long‑Term Care and community‑based nursing experience preferred.
• Excellent written, communication, and computer skills required.
• Ability to exercise independent judgment.
• Must possess and demonstrate critical thinking skills.
• Demonstrated interpersonal skills.
• Ability to develop and maintain records and communicate clinical data in verbal and written form.
• Demonstrated knowledge of the principles and practices of nursing.
• Current driver’s license (and insurance) required.
