Role Summary

Registered Nurse Case Manager (Full‑Time, 37.50 hours per week). Enjoy a better work/life balance: no evenings, weekends, holidays, or on‑call. • Determine clinical eligibility for Mass Health‑funded services for nursing facility stays.
• Complete in‑home assessments of service needs for individuals.
• Maintain and communicate clinical data in both verbal and written form.

• Registered nurse with a valid Massachusetts license.
• Bachelor of Science degree in Nursing or Associate of Science in nursing and at least one year of clinical experience in a community or long‑term care setting.
• Geriatric/Long Term Care and community-based nursing experience preferred.
• Excellent written, communication, and computer skills.
• Ability to exercise independent judgment.
• Demonstrated critical thinking and interpersonal skills.
• Demonstrated knowledge of nursing principles and practices.
• Valid driver’s license (and insurance).

Day shift – Monday through Friday. Pay range: $32.88 – $33.39 per hour.

Berkshire County – travel reimbursement provided.
