Position:
Full‑time, 40 hours per week at Harbor Community Health Center, Hyannis, MA. Salary range $115,518–$173,328 annually, with a minimum starting salary of $124,800.

The nurse practitioner provides comprehensive care to the patient population, including routine health maintenance, management of chronic conditions, and acute illness care. The role is full‑time and may include evening shifts, Saturday rotations, and holiday coverage.
Responsibilities- Serve as a primary care provider for a panel of patients
- Organize and manage patient care, including outpatient visits, communication via mail, email or phone, inpatient care, referrals, and patient education for self‑management
- Perform routine health maintenance with interval well visits, including history and physical, assessments, screening and health promotion
- Provide timely and expert care to patients with acute illness
- Manage common chronic illnesses, including complications and routine follow‑up, and provide patient education for self‑management
- Participate in an on‑call system with other providers in the Center and affiliated health centers to offer 24‑hour access for patients
- Participate in evening, Saturday and holiday clinic schedules to provide face‑to‑face care, follow‑ups, acute care and other clinical services
- Attend provider staff meetings, departmental and general staff meetings, trainings, care team meetings, and quality improvement or practice transformation sessions
- Assist with managed care plans, reinforce patient understanding, and make cost‑efficient referrals while ensuring referrals are to participating specialists
- Maintain electronic medical records (EMR) with accurate coding, progress notes, lab follow‑ups, patient communications, and current problem lists, medication flow sheets, immunization records, and other required information
- Completed accredited Nurse Practitioner program in Family Practice
- Current Massachusetts NP license, Family Practitioner; board certification; current DEA, MCSR, and Basic Life Support for Healthcare Providers (BLS) required
- Computer literate (MS Office, Windows); experienced with EMR systems, preferably EPIC/OCHIN
- Two or more years of NP experience required; bilingual preferred (English/Portuguese/Spanish)
- Experience with a community health center population and practice setting preferred
