Clinical Team Specialist

Job Overview:
Provide high‑quality home‑care services to more than 60,000 patients daily, ensuring nursing plans and documentation meet state and federal standards.

Responsibilities
  • Provide oversight of all patient care services including coordinating patient care, referrals, and ensuring needs are continually assessed.
  • Develop, implement, and update individualized patient plans of care.
  • Review and approve plans of care and evaluate proposed changes for clinical appropriateness.
  • Conduct regular OASIS reviews for all patients.
  • Ensure required visits are completed timely and that paperwork is accurate and compliant.
  • Monitor documentation timeliness, upload to HCHB, and maintain records for licensing and regulatory compliance.
  • Participate in strategic planning and QAPI activities.
  • Perform other duties and projects as assigned.
  • Work in a physically demanding, high‑stress environment.
Qualifications
  • Associate degree in a nursing‑related field.
  • At least 2 years of home‑care experience as an RN in a Home Health or Hospice environment.
  • Current, unrestricted RN license valid for the state of employment.
  • Proficiency with enterprise applications such as Homecare Homebase and Workday, plus Microsoft Office.
  • Able to travel occasionally for field‑office support; must have a dependable vehicle, valid driver’s license, and current auto insurance as required by state laws.
  • Capable of lifting, pushing, pulling, and supporting up to 50 pounds, including positioning or transferring patients and moving equipment.
  • Reporting to the Administrator or Branch Director.
Compensation & Benefits

Base salary: $85,000 to $93,000 annually, with potential performance‑based bonuses, commissions, on‑call pay, or other incentive earnings.
