DEFINITION/PRIMARY FUNCTIONThe Clinical Wellness Nurse brings a clinical foundation in health promotion, disease prevention and management to wellness interactions in community organizations, at worksites and with BHS employees. The RN intervenes to promote, maintain or restore health to prevent illness and to effectively manage the disease process or chronic condition. The RN collaborates with other healthcare providers, professionals and community representatives to achieve desired outcomes.
The RN is proactive in identifying potential health risks and intervenes quickly to minimize impact. The RN will have a strong preventive health focus. The RN develops an effective and efficient interpersonal relationship with clients and maintains a safe and supportive coaching environment. The RN fosters an environment of individual responsibility with each client. The RN evaluates the individual and family response to interventions and assists as necessary.
QUALIFICATIONS
Minimum qualifications are required unless stated otherwise.
Experience- Minimum of 1 year of acute nursing care required, hospital based preferred.
- A minimum of 6 months experience working in a “coaching” environment.
- Demonstrated knowledge of the principles and practices of nursing.
- Knowledge of and ability to apply community health principles and practice.
- Knowledge of evidence based disease management guidelines, as well as recommendations for preventive health screenings.
- Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
- Experience working in corporate or industrial setting helpful.
- Ability to work with individuals and with small groups.
- Strong assessment and communication skills.
- History of flexibility in assuming varied assignments.
- Registered Nurse required.
- Bachelor’s degree or equivalent experience in counseling / coaching for behavior change. Master’s preferred.
- Graduate of a N.L.N. accredited nursing program.
- Counseling experience preferred.
- Familiarity with corporate and worksite environments preferred.
- RN – RN License as a professional nurse in MA
- Willingness to pursue Health Coach Credential.
- Driver – Valid drivers license.
- Ability to exercise independent judgement.
- Critical thinking skills
- Demonstrated interpersonal skills.
- Ability to maintain confidential information.
- Ability to develop and maintain records and communicate clinical data in verbal and written form.
- Willing to be flexible regarding work hours.
- Ability to drive an automobile in inclement weather.
- Must be willing to accept “on call” responsibilities.
- Conforms to Standard of Gold Service/Continuous Improvement.
- Conforms to Patient’s Rights Policy.
- History of dependability in meeting demands of committed work schedule.
- CPR required every two years.

The minimum and maximum of the hourly pay range for this position is shown above. Actual rates of pay may vary based on a range of factors including but not limited to experience and qualifications.
