Must maintain emotional stability in emergency settings. Must be willing to accept on-call responsibilities. Must have a working knowledge of home care reimbursement and regulatory guidelines. Must have a current driver's license and insurance. Must have a vehicle for transportation.
Description:
The Community Health Nurse provides, teaches, supervises, and documents nursing care for the sick and injured in their homes. The nurse intervenes to promote, maintain, or restore health, prevent illness, and facilitate rehabilitation. The nurse collaborates with healthcare providers, professionals, and community representatives to coordinate care in the home setting. The nurse evaluates individual and family responses to interventions. Ability to function independently under general guidance and follow unit-specific guidelines and protocols is essential.
This role requires independent action and judgment. Utilizes time management, communication, planning, delegating, and problem-solving skills in providing care. Demonstrated interpersonal skills and the ability to supervise others are required. Confidentiality must be maintained when managing sensitive information. Ability to develop records and communicate clinical data in both verbal and written forms is necessary. Flexibility regarding work hours is important.
Ability to drive in inclement weather is necessary. History of dependability in meeting the demands of a committed work schedule is required. Requested time off during the assignment must be noted and verified upon upload. Requests for time off after submission will not be approved. Non-billable orientation of 8 hours is required. Holiday and overtime multipliers may apply. Experience with self-scheduling of caseload is preferred.
History of flexibility in assuming varied assignments is desired. A Bachelor of Science in Nursing (BSN) is highly preferred.
