LPN - Home Health

Community Health LPN

This is a 13-week contract position.

8:00 AM - 4:30 PM, 8-hour days.

The Community Health LPN provides nursing care and documentation for patients in their homes. This role involves collaboration with healthcare professionals and community representatives to coordinate at-home care and evaluate responses to interventions. The LPN is expected to function independently within the guidelines and protocols of the unit.

Responsibilities

• Provide and document nursing care for patients at home.
• Promote, maintain, or restore health and prevent illness.
• Collaborate with healthcare providers to coordinate care.
• Evaluate patient and family responses to care interventions.
• Exercise independent judgment in clinical decision-making.
• Utilize effective time management, communication, and problem-solving skills.
• Maintain emotional stability in emergency situations.
• Adhere to patient rights policy and customer service standards.

Required Experience / Certifications / Licensure

• Current Massachusetts LPN license.
• One year of acute nursing care experience; prior hospital-based experience preferred.
• Experience in home health care required.
• Certification in Home Health preferred, including proficiency with OASIS.
• Current Driver License and insurance are required.
• Ability to drive in inclement weather and accept on-call responsibilities.
