Licensed Practical Nurse; LPN - Long Term Care

Community Health LPN

Schedule:

8:00 AM - 4:30 PM, 8 hour days

Assignment Details:

• Contract Length: 13 weeks

Requirements:

• Current Massachusetts LPN license
• Active American Heart Association BLS or American Red Cross Certification
• One year of acute nursing care experience required; hospital-based experience preferred
• Experience in home health is required
• Certified Home Health experience including proficiency with OASIS is highly preferred
• Current driver's license and insurance are required
• Experience with self-scheduling of caseload preferred

Roles & Responsibilities:

• Provide and document nursing care for patients at home.
• Collaborate with healthcare providers to coordinate care for individuals and families.
• Evaluate and respond to patient and family needs and interventions.
• Function independently within established guidelines and protocols.
• Demonstrate effective time management, communication, and critical thinking skills.
• Maintain confidentiality of patient information and develop clinical records.

Compliance / Notes:

• Must include agency Massachusetts Temporary Nursing Service Agency License with submissions.
• Will not consider any candidate residing within 50 miles of the facility.
