Licensed Practical Nurse (LPN) - Long Term Care

Role: Licensed Practical Nurse (LPN) specializing in Long Term Care, providing home health nursing services.

• Provide and document nursing care for patients in their homes.
• Collaborate with healthcare providers to coordinate patient care.
• Assess and respond to patient and family needs with appropriate interventions.
• Operate independently within established guidelines and protocols.
• Maintain confidentiality and develop accurate clinical records.
• Demonstrate effective time management, communication, and critical thinking skills.

• Current Massachusetts LPN license.
• Active BLS or Red Cross certification.
• At least one year of acute nursing care experience, with hospital and home health experience preferred.
• Proficiency with OASIS and certified home health experience highly preferred.
• Valid driver's license and insurance required.
• Experience with caseload self-scheduling is a plus.

Ability to work scheduled days, adhere to compliance standards, and include necessary licensing documentation. Ability to work independently with strong communication and organizational skills.
