Licensed Practical Nurse (LPN) In Home Health
The Licensed Practical Nurse (LPN) in home health provides and documents nursing care for patients in their homes. They assess, plan, and implement clinical interventions to promote health, prevent illness, and support rehabilitation under general supervision.
Responsibilities:
- Administer nursing care and interventions tailored to individual patient needs.
- Collaborate with healthcare team members and community resources to ensure coordinated care.
- Evaluate patient responses to treatments and modify care plans accordingly.
- Maintain accurate documentation of clinical data, assessments, and care provided.
- Follow established protocols, policies, and safety guidelines in all nursing activities.
- Exercise independent judgment and critical thinking in emergency and routine situations.
Qualifications:
- Possess a current Massachusetts LPN license and include it with profile submission.
- Demonstrate at least one year of acute care nursing experience, preferably hospital-based.
- Have home health care experience with proficiency in OASIS data.
- Maintain a valid driver's license and own vehicle for travel between patient homes.
- Hold current American Heart Association BLS or Red Cross certification.
- Be flexible regarding work hours and on-call responsibilities.
