Home Health LVN / LPN

The Home Health LVN / LPN provides skilled nursing care to patients in their home environment, promoting health, preventing illness, and supporting rehabilitation efforts under minimal supervision.

Responsibilities:

• Administer prescribed medications and treatments according to established protocols.
• Document patient assessments, interventions, and progress accurately in clinical records.
• Collaborate with healthcare team members and community providers to coordinate care.
• Educate patients and families on health management, medication adherence, and safety.
• Respond promptly to patient needs and emergency situations while maintaining emotional stability.
• Follow all household, safety, and regulatory guidelines, including documentation and confidentiality standards.

Qualifications:

• Valid Massachusetts LVN / LPN license with active certification.
• Minimum of one year of acute or home health nursing experience.
• Demonstrate proficiency with OASIS and infusion therapy, preferred.
• Possess a valid driver's license and reliable vehicle, with insurance.
• Ability to perform independently with excellent clinical judgment and problem-solving skills.
• Strong interpersonal and communication skills, with the ability to maintain confidentiality.
