Community Health Nurse (CHN)
The Community Health Nurse (CHN) is responsible for providing, supervising, and documenting nursing care to patients in their homes. This role emphasizes promoting health, preventing illness, and supporting rehabilitation through independent nursing interventions and collaborative care.
Responsibilities:
- Deliver comprehensive home health nursing services adhering to established protocols.
- Assess patient and family needs, developing and implementing personalized care plans.
- Collaborate effectively with healthcare professionals and community partners to coordinate care.
- Document clinical interventions, patient responses, and progress accurately.
- Exercise independent judgment and critical thinking in emergency and routine situations.
- Manage time efficiently, including self-scheduling and prioritizing caseloads.
Qualifications:
- Possess a valid Massachusetts RN license and maintain current BLS certification.
- Have at least one year of acute hospital-based nursing experience, with home health experience preferred.
- Demonstrate proficiency with OASIS and infusion therapy certification is a plus.
- Exhibit strong interpersonal, communication, and supervisory skills.
- Be flexible with work hours, including on-call responsibilities, and able to drive in inclement weather.
- Maintain a clean driver's license and own a vehicle for travel.
