Community Health LPN - Home Health Nursing
The Community Health LPN provides nursing care for the sick and injured in a home setting. This role involves promoting, maintaining, and restoring health, preventing illness, and facilitating rehabilitation. The LPN will work collaboratively with other healthcare providers and community representatives to coordinate care and evaluate the responses of individuals and families to interventions.
Responsibilities- Provide and document nursing care for patients at home.
- Collaborate with healthcare professionals to coordinate patient care.
- Evaluate patient responses to interventions and adjust care plans as necessary.
- Utilize time management and critical thinking skills to problem solve and deliver patient care effectively.
- Maintain emotional stability in emergency situations.
- Meet patient care standards and uphold patients' rights.
- Accept on-call responsibilities as needed.
- One year of acute nursing care experience, hospital-based preferred.
- Experience in home health is required; proficiency with OASIS is highly preferred.
- Certified Home Health Nursing certification is required.
- Infusion therapy experience is preferred.
- Current Massachusetts LPN license required.
- Active American Heart Association BLS or American Red Cross certification is required.
- Driver's license and vehicle for travel to patient homes are mandatory.
This position requires an 8:00 AM to 4:30 PM schedule for 8-hour shifts, totaling 40 hours per week.
