Description:
The Community Health Nurse (CHN) provides, teaches, supervises, and documents nursing care of the sick and injured in their homes. The CHN intervenes to promote, maintain, or restore health to prevent illness and effects rehabilitation. The CHN collaborates with other healthcare providers and community representatives to coordinate care CHN evaluates the individual's and family's response to interventions. The role requires independent action and judgment.
Effective time management, communication, and problem-solving skills are essential. Candidates must demonstrate emotional stability in emergency settings. Ability to exercise independent judgment is required. Candidates must maintain confidentiality and develop clinical records. It is necessary to have a willingness to accept on-call responsibilities. The CHN must conform to customer service and performance standards. A history of dependability in meeting work schedule commitments is important.
Any requested time off during the assignment must be noted upfront. The Community Health Nurse should possess knowledge of home care reimbursement and regulatory guidelines. Participants in this role must have a working knowledge of community health principles and practices. Experience with self-scheduling of caseload is preferred. A Bachelor of Science in Nursing is highly preferred. Traveler candidates must have their own vehicle to perform home care duties effectively.
