Description:
Requested time off during the course of this assignment must be noted and verified uests after upload or during interview will not be approved.
No more than five days of requested time off will be accepted; less is better.
Please do not submit candidates who would like to work in conjunction with another permanent placement or travel assignment.
Candidates must work two blocks during specified holidays if scheduled/needed.
Candidates residing within 50 miles of the facility will not be considered.

A supervisory signed reference from within the past two years of employment is required.
Current active licenses and certifications must be uploaded in the profile.
