Youth Outreach Case Worker

18 Degrees is seeking a Youth Outreach Case Worker to bridge youth support and digital product work within our Northampton-based studio. In this role, you'll engage young people through outreach, intake, and ongoing support, using modern digital tools to manage cases and stay connected. You'll coordinate with schools, community partners, and families while documenting needs, goals, and progress. Working closely with our product and engineering teams, you'll share insights from youth to help shape user-centered web and mobile experiences.

This hybrid role suits a hands-on practitioner who values thoughtful design, collaboration, and continuous learning.

Responsibilities

• Engage youth clients through outreach, intake, and ongoing check-ins to understand goals, challenges, and support needs
• Use digital tools, case management software, and communication platforms to track cases and coordinate services
• Collaborate with product and engineering teams to share youth insights that inform digital tools and program features
• Develop and maintain individualized service plans, documenting progress and adjusting supports as needs evolve
• Coordinate with schools, community organizations, and families to connect youth with resources and opportunities
• Facilitate small group sessions, workshops, or virtual meetups focused on digital literacy, career readiness, or wellness
• Monitor engagement metrics across web and mobile platforms to identify at-risk youth and outreach priorities
• Maintain accurate, timely records in line with privacy, ethical, and organizational standards
• Participate in team standups, retrospectives, and learning sessions to continuously improve outreach practices
• Contribute to a collaborative, learning-focused culture by sharing observations, feedback, and ideas for program improvement
