Key Responsibilities:
  • Assess patients and develop individualized treatment plans to improve movement and function

  • Document patient progress and collaborate with interdisciplinary healthcare teams
  • Ensure compliance with facility policies and state/federal regulations
    • Qualifications:
  • Active Physical Therapist (PT) license in CA
  • Experience in skilled nursing, rehabilitation, or long-term care preferred
  • Strong communication, patient-care, and teamwork skills
