The Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) will collaborate with the attending physician to provide comprehensive, patient-centered care to individuals in post-acute and long-term care settings. This role focuses on managing chronic and rehabilitative conditions, ensuring continuity of care across multiple sites, and supporting interdisciplinary teams. The ideal candidate is comfortable working with geriatric populations and is willing to participate in an on-call rotation.
Key Responsibilities Clinical Patient CareAssist attending physicians in managing patients in post-acute and long-term care settings.
Evaluate, diagnose, and treat patients during rehabilitation and recovery phases.
Monitor and manage chronic medical conditions.
Perform routine assessments, medication management, and follow-up visits.
Collaborate with nursing staff, therapists, and interdisciplinary teams to ensure coordinated care.
Provide timely clinical interventions to support optimal patient outcomes.
Site Coverage & TravelTravel between multiple care sites within the Greater Sacramento region.
Provide clinical coverage based on patient census and organizational needs.
Ensure continuity and consistency of care across all assigned locations.
On-Call ResponsibilitiesParticipate in an on-call rotation as scheduled.
Respond to urgent patient needs during assigned on-call periods.
Support after-hours clinical decision-making under physician guidance.
QualificationsActive California Nurse Practitioner (NP) or Physician Assistant (PA) license.
Board certification as an NP or PA.
Minimum 1 year of post-graduate provider experience.
Experience in Skilled Nursing, Post-Acute Care, Long-Term Care, or Geriatrics.
Reliable transportation for multi-site coverage.
Strong clinical judgment and decision-making skills.
Excellent communication and teamwork abilities.
