We are seeking an experienced
** LVN Wound/Treatment Nurse
** who thrives in a collaborative environment and is committed to providing exceptional resident care. Our supportive leadership team and positive workplace culture offer opportunities for continued learning, advancement, and career growth.## What You'll DoAs our Wound/Treatment Nurse, you will play a vital role in maintaining and improving residents' skin integrity and overall well-being.
Responsibilities include:
* Assessing, monitoring, and treating wounds, skin conditions, and other dermatologic concerns
* Developing and implementing individualized treatment plans in collaboration with physicians and the interdisciplinary care team
* Performing skin assessments and maintaining accurate clinical documentation
* Monitoring treatment effectiveness and recommending care plan adjustments as needed
* Providing resident and family education regarding wound prevention and treatment
* Ensuring compliance with infection control practices, regulatory standards, and facility policies
* Participating in care planning meetings and collaborating with nursing staff, physicians, therapists, and other healthcare professionals
* Assisting with admissions, discharges, rehabilitation support, and resident advocacy as needed
* Helping maintain adequate wound care supplies and treatment resources## Qualifications
* Current, active, and unrestricted California LVN license
* Wound Care Certification required
* Previous wound care and treatment nursing experience required
* Minimum of two (2) years of nursing experience
* Strong assessment, documentation, and communication skills
* Ability to work effectively as part of a multidisciplinary team
