Saint Joseph Medical Center in Joliet, IL, is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Family Practice team at our Primary Care clinic in Plainfield, Illinois
.

This full-time role offers an excellent opportunity to provide comprehensive outpatient care in a collaborative and community-focused setting.

• Will be employed by Prime Healthcare Illinois Medical Group
• Full time | Dayshift schedule, 8 am- 5pm (flexible)
• Deliver full-spectrum primary care to patients of all ages
• Manage chronic conditions, perform preventative care, and address acute health concerns
• Work closely with physicians and clinical staff to ensure coordinated, high-quality care
• Maintain accurate and timely documentation in the electronic medical record

• Licensed NP or PA in the state of Illinois
• Board certification in Family Medicine (NP) or appropriate PA certification
• Strong interpersonal and clinical skills
