Residential Home Health - - Responsibilities:
Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist;
Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support;
Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team;
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation;
Travel to patient homes within assigned territory
