Provide skilled Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist;
Help patients restore mobility, reduce pain, and maximize functional independence through therapeutic exercises and gait/transfer training;
Educate patients and caregivers on safe mobility and device use and monitor progress;
Maintain thorough EMR documentation and communicate patient responses to the care team;
Travel to patient homes within assigned territory
